I acknowledge that it is my sole personal responsibility to assess whether any competition or training course, respectively the conditions there of at the relevant time are too challenging for me.

I hereby declare that I have no physical oe mental condition that should preclude me from participating in my chosen activity, that I am not participating against medical advice or treatment.

I further declare that in the event that I feel ill or unwell, have any physical complaints whatsoever or if any injury is sustained of any kind during the course of senior Sports Day activities, I will notify a volunteer on duty, immediately and before leaving the premises. I release Amrit sagar Foundation and their respective members and volunteers as well as any other entities and persons involved with or participating the Senior Sports Day event from any liability including loss, injury, damage and costs incurred or suffered in relation to my participation in the senior Sports Day event.
